Improve the current situation in community care for chronically ill patients which are dependent upon help of others.

Programme, project, innovation description
In frame of this project Integrated Community Care is understood as an effective integration and co-ordination of primary health services and social services and public health services provided on the community level. The aim of the project is to improve current situation in community care for chronically ill patients which are dependent upon help of others in two socially disadvantaged areas Kladno and Slavkov. The target groups are defined as follows: Chronically ill patients, elderly, handicapped people, children with serious chronic illness. The emphases will be put on availability, quality and cost-effectiveness of provided community services (as defined above) . The special attention will be paid to the role of primary care providers and their links to secondary health care and to social care. Project is split into two parts: pilot and implementation ones. In a pilot phase needs assessment of selected patients from the point of view of users themselves, their relatives and caregivers, physicians, home care nurses, social workers, other health professionals and local authorities will be implemented.
Expected outcomes in this phase:
A complex description of a real situation in provision of community care in both selected areas.
Identification of the weaknesses in co-ordination of existing services as well as identification of needed but missing services.
Questionnaire surveys and focus groups interviews will be used for collecting needed information. Setting up the key points for effective intervention will follow this phase of project.
In the implementation phase new innovative model of co-ordination of community services will be proposed and implemented in some extend into the practice. Changing of educational curricula for the key health personnel to be prepared to work more effectively in accordance with real needs of above mentioned groups of patients. Evaluation of impact of project implementation will be performed through a measurement of users satisfaction and their quality of life, job satisfaction of the key providers in the community, and related cost-effectiveness analysis.
